Vision StatementOlder persons with one or more chronic illnesses will be assisted by a professional aide/agency to plan and arrange use of residential and health/social services suited to maximizing independent functions (physical, social, spiritual and emotional) and to pursue individual aspirations throughout their lives by entering into a relationship with a network of providers organized to support these objectives to the best of their ability. Mission StatementTo provide older persons, directly or in concert with their spouse and family members, access to an array of high-quality residential and health-related service options while achieving continuity of care and a high degree of consumer satisfaction through a program of professional care management. Catholic hospitals, nursing homes, social service and housing organizations will manage a collaborative effort to arrange a network of services across six counties and offer a means for single central access with optional case management supervision for those with such need. Care for Life, a program to help the older adult maintain health and well-being, independence and optimal quality of life, is an initiative of Catholic health and social service providers in the Diocese of Albany.
